The CPP functions as part of the HBPC care teams and is most effective when assigned to patients to support continuity of care. Care will be provided through a variety of modalities including, but not limited to, telephone, virtual care, chart consults and in-home visits. This position will be assigned to the Marquette, MI and Gladstone, MI area. The nurses are available 6 days a week and work with their own nurse practioners. VA has opened over 700 community-based outpatient clinics to increase Veterans’ access to care, making us world leaders in telehealth.

The Home Based Primary Care Physician must have medical practice licensure form the Commonwealth of Virginia. Ability to obtain licensure in the District of Columbia and State of Maryland is a plus. Must also have DEA and State Controlled Substances Registration in the listed jurisdictions. The Home Based Primary Care Physician must have a MD or DO from an accredited institution. American Reg of Radiologic Tech or American Reg of Diagnostic Med Sonograph or Cert Nuc Med Tech or Cert Rad Admin required based on specialty.

I know they have Health Buddy telehealth which is also what my agency uses. They have all modules, I think..which are DM, COPD, and CHF. Recently, I spoke to a social worker about the VA and told that they deal with quite a bit of psych issues in the home based program. I know that the VA is big on following the standards of care and working with clinical guidelines so I would be those things would be incorporated into their delivery of care. The vets I've dealt with in my own agency have been pretty high acuity patients, ie wound care, DM.
